In 2018 we combined the ADA 2012 OCR template with the ADA 2006 version to create a single, flexible recognition template that identifies both forms simultaneously. Since the majority of the data fields on the two versions of the form are identical, it is better for training and maintenance to have a single workflow for both forms. Questions and check-boxes that appear in different positions on each form are located dynamically using a rules-based template to identify the data rather than fixed coordinates like other OCR solutions use.

The ADA Dental Claim Form has been revised to incorporate key changes to the HIPAA standard electronic dental claim transaction. Changes to the form include the reporting of diagnosis codes and diagnosis code pointers, place of service codes, and other medical and dental coverage. Every dentist should be using the 2012 version of the ADA Dental Claim Form to avoid delayed payment and claims processing problems.
